- 4 photos of 3×4 cm in size;
- medical certificate on form No. 075-U, conclusion of medical and social examination on form No. 031-U for disabled people of I and II groups and disabled from childhood;
- identity document (for identity verification).
Documents for applicants for an applied bachelor’s program on the basis of post-secondary, higher (higher professional) medical education:
- Application (according to the applicant’s sample form)
- Original document on education
- A copy of the work book certified by the head of the human resources management service (at least 3 years of work experience in the specialty “Nursing”, “Medical” with the qualification of “Paramedic” in the specialty “Nursing”)
- Medical certificate on form #075-U
- Specialist certificate in the specialty “Nursing”.
- 4 photos of 3×4 cm in size;
- Identity document (for identification)
The study period is 1 year and 6 months (according to the accelerated program), he receives the qualification “Applied Bachelor”.
